Independence is the point of the work

Valley Coordinated Services mission is to ensure that all individuals supported receive valuable and resourceful services that will enrich their quality of life and promote independence.

Our vision is to empower, embolden and build an enhanced, integrated everyday life for as many individuals as possible from of all walks of life.

Who we serve

Valley Coordinated Services supports people who receive Pennsylvania Office of Developmental Programs (ODP) home and community-based services, and the families and coordinators who plan that support.

People seeking an everyday life

We support individuals with intellectual disabilities, autism, and related developmental disabilities who want help at home, in the community, and on the job. The work is person-centered: the ISP drives where we show up and what we practice.

Families and guardians

Parents, siblings, and guardians use us when they need staff who will actually keep hours, communicate, and treat the person's home as the person's home. You stay part of the team.

Supports coordinators

SCs refer when they need a provider for Community Participation Support, In Home and Community Support, Companion, or ongoing job support in our counties. We take referrals by phone and email and will tell you quickly if we can staff the authorization.

Counties, services, and eligibility

Four ways we show up with you

Direct support in the community, at home, as a companion, and on the job. Each service is built to inform the plan, prove it can work, and get a referral moving.

Community participation support

Community Participation Support (CPS) gets people out into real places: shops, parks, classes, and neighborhood life. Staff stay with the person so they can try new settings, meet people, build interests, and practice the skills that keep them safe.

A full week is not built in a waiting room. Families and supports coordinators choose CPS when isolation is the problem and independence is the goal. We provide 1:1 and 2:1 community hours as a Pennsylvania ODP enrolled provider in Berks, Bucks, Lehigh, Montgomery, and Northampton counties.

If community hours on the plan are not actually happening, ask your supports coordinator about a referral to Valley Coordinated Services. Call 610-956-9951 and we will walk you through the next step.

In home and community support

In Home and Community Support is planned around one person at a time. Staff help with choices, routines, and daily tasks at home and out in the community so the person stays in charge of their day.

Generic hours waste a waiver. This service is for people who need hands-on guidance that follows their plan, not a one-size schedule. We keep the focus on independence, health, and the skills that make home life work.

If home support has been hard to staff, or the plan on paper is not showing up in the kitchen, talk with us. We will review the referral and tell you what we can start.

Companion services

Companion services put a trained staff person in the home for supervision and help with activities of daily living, with close attention to health and safety needs.

This is the right fit when someone needs presence and steady help, not a clinic. A companion keeps the day moving, notices problems early, and supports dignity without taking over the person's home.

Families who need reliable coverage at home can call the Allentown office. Ask about companion hours and we will confirm next steps with your supports coordinator.

Employment services

Employment services provide ongoing job support for people in competitive work. Staff help keep performance, skills, and habits on track, solve problems that could cost the job, and teach new tasks as the workplace changes.

A job is one of the strongest paths to an independent adult life. Losing one is hard to reverse. We stay close enough to catch issues before they become a write-up or a lost position.

If someone you support is working, or ready to work, ask us about job support. Call 610-956-9951 and we will talk through what on-site help can look like.

How to start

If you already have a supports coordinator, ask them to refer you here. If you are new to ODP, the county office is the first call.

  1. Reach your county MH/ID office

If you are not yet in the ODP system, start with your county Mental Health / Intellectual Disability office. They handle intake, eligibility, and choice of a Supports Coordination Organization. Bring records that help show need, such as medical, school, or psychological documents.

  2. Work with a supports coordinator

Once eligible, you choose a Supports Coordination Organization. Your supports coordinator helps write the Individual Support Plan (ISP) and connects you with providers. You can change SCOs later if the fit is wrong.

  3. Ask for Valley Coordinated as a provider

If you already have a supports coordinator, tell them you want Valley Coordinated Services for community participation, in-home support, companion, or employment hours. Provider choice belongs to the person and family.

  4. Send the referral

Supports coordinators can call 610-956-9951 or email info@valleycoordinated.com. Helpful items include the person's name, county, authorized services, ISP or service notes, and a family contact. We will confirm what we can staff.

  5. Intake and start of hours

We review the referral, talk with the team, and set up staffing. Hours begin after authorizations are in place. Direct support often happens outside office hours. The Allentown office is open Monday to Friday, 9:00 am to 5:00 pm for calls.
