Privacy and rights

This page covers how this website uses information, the rights people have when they receive our services, and how to report a concern. It is a public summary, not a substitute for the written notices given at intake.

Website privacy

The contact form collects the name, email, how you heard about us, and any message you type. We use that information to reply about services or jobs. We do not sell website form data.

If you send health, waiver, or ISP details by email or form, treat that as a business communication with a provider. Do not use the public form for emergencies.

Rights of people we support

People who receive ODP-funded services have the right to an everyday life, to be treated with dignity, and to be protected from harm. In plain terms, that includes the right to:

  • Be treated with respect in your home and in the community
  • Privacy of your body, your space, and your information
  • Help that follows your Individual Support Plan
  • Choose and change providers
  • Communication you can understand
  • Freedom from abuse, neglect, and exploitation
  • Take part in planning your services
  • See records about you, as allowed by law
  • File a complaint without being punished for speaking up

Staff must not intimidate, discriminate against, or retaliate against anyone who reports a concern or helps with an investigation.

If something goes wrong

If someone is in immediate danger, call 911. Then tell the office.

ODP providers must recognize, report, and respond to incidents that affect health, safety, or rights. That includes suspected abuse, neglect, or exploitation. Reports go through Pennsylvania's incident system. Families and people designated in the ISP are to be kept informed, and may request a summary of what was found and what was done.

Tell us at 610-956-9951 or info@valleycoordinated.com. You may also contact your supports coordinator, your county Intellectual Disability program, or ODP customer service at 1-888-565-9435. TTY 1-866-388-1114.

Complaints about this agency
