How to start with us
Pennsylvania ODP support has two gates: county eligibility, then a provider the person actually wants. Use this page to find your place in that path and send a clean referral.
Most referrals
You already have a supports coordinator
Tell your SC you want Valley Coordinated Services. They send the referral. We confirm we can staff, then hours can start once the authorization is in the ISP.
Go to the SC path
First time in the system
You are not in ODP yet
We cannot put you on a waiver from this website. The county MH/ID office does intake. Call us anyway if you want to know whether our services fit, then come back with your SC.
Go to county intake
Already have a supports coordinator
This is the fastest path. Provider choice belongs to the person and family. Your SC writes Valley Coordinated into the plan and sends us the packet. We answer on capacity before anyone pretends hours are staffed.
Say this to your SC: we want Valley Coordinated Services for community, in-home, companion, or employment hours in Berks, Bucks, Lehigh, Montgomery, or Northampton County.
What to send with the referral
Incomplete packets slow start dates. Send what you have. We will ask for the rest.
- Person's legal name and preferred name
- County of residence
- Supports coordinator name and phone
- Requested service: CPS, in-home, companion, or employment
- Authorized units or frequency, if already approved
- ISP pages or service notes that describe goals and risks
- Family or guardian contact
- Schedule windows that actually work in the home

New to ODP in Pennsylvania
Home and community-based waiver services start at the county, not at the provider. The sequence below is the same one DHS and other ODP providers describe: county intake, eligibility, a supports coordinator, an ISP, then a provider you choose.
Reach your county MH/ID office
If you are not yet in the ODP system, start with your county Mental Health / Intellectual Disability office. They handle intake, eligibility, and choice of a Supports Coordination Organization. Bring records that help show need, such as medical, school, or psychological documents.
Work with a supports coordinator
Once eligible, you choose a Supports Coordination Organization. Your supports coordinator helps write the Individual Support Plan (ISP) and connects you with providers. You can change SCOs later if the fit is wrong.
Ask for Valley Coordinated as a provider
If you already have a supports coordinator, tell them you want Valley Coordinated Services for community participation, in-home support, companion, or employment hours. Provider choice belongs to the person and family.
Send the referral
Supports coordinators can call 610-956-9951 or email info@valleycoordinated.com. Helpful items include the person's name, county, authorized services, ISP or service notes, and a family contact. We will confirm what we can staff.
Intake and start of hours
We review the referral, talk with the team, and set up staffing. Hours begin after authorizations are in place. Direct support often happens outside office hours. The Allentown office is open Monday to Friday, 9:00 am to 5:00 pm for calls.

After we get the referral
Someone in the Allentown office reviews the packet and calls back. We check county, service type, schedule, and whether we have staff who can do the work well.
If we can take it, we join the team conversation, complete intake, and match staff. Hours start only after the authorization is active. Direct support often happens in evenings and weekends even though the office phone is weekday 9:00 am to 5:00 pm.
If we cannot staff it, we say so. That keeps the ISP honest.

Common questions
- Can I call Valley Coordinated before I have a supports coordinator?
- Yes. We can explain our services and which counties we cover. We cannot enroll you in a waiver. Eligibility and SCO choice start at the county MH/ID office.
- Who decides that we are the provider?
- The person and family choose the provider. The supports coordinator writes that choice into the ISP and sends the referral. We then confirm we can staff the hours.
- How long until hours start?
- It depends on authorizations, the schedule, and staffing. We will say quickly if we can take the referral. Hours do not start until the authorization is in place.
- Do you serve people who are not on a waiver yet?
- Our listed services are ODP home and community-based supports. If you are still in county intake, we can still talk, then you loop us in once the SC is assigned.
- What if you cannot staff the hours?
- We will say so. It is better to know before the ISP is written around a provider who cannot show up.
